The prognostic significance of bone marrow histological evaluation in patients with multiple myeloma

Introduction: Recently, there has been enormous progress in the diagnosis and treatment of multiple myeloma (MM). Despite importance flow cytometry cytogenetic, biomarkers are being sought to assess prognosis response treatment. The aim this study was determine significance histological evaluation immunohistochemical markers MM patients. Material methods: A retrospective analysis performed 90 bone marrow (BM) biopsies collected during period related recognized prognostic factors standardized staining methods monoclonal antibodies were used detect cluster differentiation antigens — CD138, CD56, CD20, CD117, cyclin D1, p53, kappa lambda light chains. Results: During follow-up 49.6 (2–119.7) months patients with increased levels β 2 -microglobulin, higher International Prognostic Classification stage, resistance induction relapse rate after first-line, a clonal plasmocytes infiltration observed. It also affected immunomodulatory drugs (IMiDs), but not bortezomib. lowest IMiDs noted CD117-lambda+ CD56-lambda+ plasma cells. Clonal hyperplasia connected reduction overall survival [hazard ratio (HR) 2.4; 95% confidence interval (CI): 0.96–5.92; p =0.047) progression-free (PFS) (HR 4,32, CI: 1.9–9.5; =0.0002). presence phenotype 3.37; 1.57–7.24; =0.002) 3,44; 1,63–7,27; =0.001) contributed PFS shortening. Conclusions: In conclusions, BM biopsy immunohistochemistry allow identification poorly MM.
